Your presentation on the difficult of using the Fishbone is logically illustrated with the example that you chose.
The problem statement should give you strong clarity as to what exactly needing to be investigated. Weak statement can create that sense of unclarity as to what you should be examining, comparing or measuring.
You mention that another document is needed when utilising the Fishbone but you left the readers hanging by not giving the name of the document(s) 5 Whys, Root Cause etc.
Overall I like it and the improvement actions given on it.
